SALT LAKE CITY--(BUSINESS WIRE)--Eccovia Solutions announced today that it has been selected by the Los Angeles County Department of Health Services to provide the integrated care coordination platform for the County’s five-year 1115 Waiver Whole-Person Care (WPC) pilot program.
The goal of the Los Angeles County WPC pilot is to build an integrated, countywide health system that delivers coordinated wrap-around services to approximately 50,000 of the most vulnerable and high-risk individuals living in Los Angeles County. The program will improve their health and their care experience, and decrease the use of high cost services. Los Angeles County is the largest county by population in the United States and has the second highest number of individuals experiencing homelessness. Eleven innovative programs will bring together health and social service delivery entities across the county to provide seamless, coordinated services to individuals who are homeless or justice-involved, or have serious mental illness or severe and/or persistent substance use disorder or medical issues, including:
- Homeless Care Support Services
- Tenancy Support Services
- Recuperative Care
- Sobering Center
- Benefits Advocacy
- Re-entry Enhanced Care Coordination
- Community-based Re-entry
- Intensive Service Recipients
- Residential and Bridging Care
- Substance Abuse Disorder Engagement, Navigation, and Support
- Transitions of Care
The ClientTrack Care Coordination platform provided by Eccovia Solutions enables states, counties, Accountable Care Organizations (ACOs), and other Health and Human Services (HHS) organizations to bridge the gap between primary care and community services by incorporating the social determinants of health into healthcare decisions and securely sharing patient information. ClientTrack’s flexible and specialized platform enables providers to collaborate across multiple agencies and provides the data visualization to measure and report outcomes and meet complex regulatory requirements.
