Puzzle Healthcare Receives Best Care/Disease Management Recognition from eHealthcare Leadership Awards

Company celebrates its first award highlighting the company’s continued commitment to transforming post-acute care

This eHealthcare Leadership win comes directly after Puzzle’s recent announcement celebrating their successful partnership with OSF HealthCare, a non-profit integrated health care network serving patients of all ages across Illinois and Michigan. OSF HealthCare identified an urgent need to address gaps in transitions of care between care settings where patients discharged from hospitals to skilled nursing facilities (SNFs) were being readmitted at a rate of 29% due to complications that could have been prevented. With better monitoring and more proactive management that Puzzle Healthcare’s program offers, OSF was able to lower readmission rates from 29% to 9%.
