Industry’s leading post-acute analytics solution highlights how ACOs, hospitals, health systems, and payers are utilizing live data to effectively manage their post-acute care networks, achieving a 52% reduction in readmissions while also improving value-based care outcomes

BALTIMORE, Md.--()--Real Time Medical Systems (Real Time), the industry leading, KLAS Rated, and HITRUST Certified post-acute analytics solution, announced today its participation at the 2024 Fall Managed Care Forum, on November 7-8, 2024 in Las Vegas, NV. Joining healthcare executives from the American Association of Integrated Healthcare Delivery Systems (AAIHDS), the American Association of Managed Care Nurses (AAMCN), and the NAMCP Medical Directors Institute, the Real Time team will be sharing how live data analytics is empowering care teams throughout the continuum to achieve value-based goals and successes. By leveraging post-acute data, providers are decreasing avoidable rehospitalizations, identifying early signs of infectious diseases, strengthening care coordination efforts, and enhancing patient outcomes.

Contributing to the Fall Managed Forum’s educational agenda, Real Time’s Phyllis Wojtusik, Executive Vice President of Value-Based Care, will explore the “why” and “how” care coordinators can effectively leverage post-acute data to facilitate smooth care transitions, establish clinical pathways, monitor high-risk patients, lower length of stay, reduce readmissions, and increase network referrals. The session, Managing the Post-Acute Network: Care Coordinators + Live Data Analytics = Value-Based Care Success, will take place on Thursday, November 7, 2024, from 3:00pm – 4:00pm PT at the Caesars Palace Hotel & Convention Center, where attendees will have the opportunity to share challenges and ask questions.

“I’m looking forward to discussing the pivotal role data transparency plays in transforming care delivery,” states Phyllis Wojtusik, RN. “By sharing best practices and lessons learned on prioritizing patients in clinical need, alerting to necessary interventions, and enhancing overall quality of care, providers can strengthen care coordination efforts through live post-acute data transparency – empowering them to effectively drive value-based care initiatives alongside their post-acute networks.”
