Automated and hyper-personalized digital outreach activated hard-to-reach patients to close care gaps and improve health outcomes while reducing administrative hours and saving millions in operating costs for the South Carolina-based health system.

Published by the American Association for Physician Leadership (AAPL), the journal showcases interdisciplinary results-oriented guidance to health professionals, anchored on population health outcomes. The partnership, established in 2020, began with Prisma Health deploying digital outreach via Upfront to achieve better activation among their patient population. The program has since expanded to drive care orchestration across prescription refills; medication adherence; and preventive care gap recall for annual wellness visits, A1C diabetes management, diabetic eye exams, diabetic kidney disease, colon and breast cancer, and routine vaccines. For Prisma Health, automated and personalized outreach has driven both operational and clinical outcomes, saving more than 392,000 full-time employee hours to date while closing critical care gaps and activating 50% of previously nonadherent patients to refill their prescriptions.
