Included Health Earns URAC Accreditation for Case Management

Designation demonstrates highest level of commitment to quality healthcare through collaborative care and patient advocacy

SAN FRANCISCO--()--Included Health, a new kind of healthcare company delivering personalized all-in-one healthcare to millions of people nationwide, is proud to announce it has, once again, earned URAC Case Management (CM) accreditation. This recognition underscores Included Health’s ongoing commitment to delivering high-quality care, enhancing patient engagement, and advancing collaborative care. This is the second time Included Health has received URAC accreditation for Case Management; adding to its existing URAC accreditation in Telehealth.

Included Health’s approach to Care and Case Management is built on the principle of providing whole-person care with a multidisciplinary team that addresses both medical and behavioral health needs. By focusing on high-quality comprehensive care, Included Health aims to improve health outcomes and reduce the total cost of care for its most complex and high-acuity members. With this foundation, Included Health’s Care and Case Management program is proven to drive impactful results:

  • 17% Reduction in Hospitalization: Members enrolled in Care and Case Management have seen a significant reduction in hospital admissions, highlighting the effectiveness of proactive, coordinated care.
  • 19% Reduction in Urgent Care Visits: The program’s focus on preventive care and timely interventions has also led to a notable decrease in urgent care visits.

“URAC accreditation shows that Included Health adheres to best-in-class industry standards for clinical excellence, collaborative care, patient empowerment, risk management and organizational leadership and accountability. With this distinction, Included Health demonstrates excellence in quality health care delivery and their commitment to ensuring patient safety,” said URAC President and CEO Shawn Griffin, M.D.

Included Health’s dedication to clinical engagement is further highlighted by its Healthy Days program, which assesses and enhances members' well-being, many of whom are enrolled in the company’s Care and Case Management program

  • >44,000 Members Engage: Among those who completed at least two Healthy Days surveys, 32% met the outreach threshold, reporting an average of 15 or more unhealthy days each month.
  • 1/3 Accept Clinical Outreach: Of the members flagged for outreach, 33% accepted offers for clinician support, demonstrating a strong willingness to engage in their health management.

"At a time when people are increasingly avoiding phone calls,1 Included Health members are actively engaging with their healthcare,” said Ami Parekh, M.D., J.D., chief health officer at Included Health. “Navigating healthcare is challenging, but with the right infrastructure and support, we can simplify the experience and deliver improvements in health outcomes and cost savings. Our interdisciplinary teams provide personalized care plans that are proven to make a difference, bringing a new kind of value to patients, families and to our clients.”

URAC accredits outcomes-driven health care organizations who exemplify excellence in healthcare delivery. Through URAC’s CCM accreditation, Included Health has committed to ongoing clinical oversight of medical and behavioral health program goals and metrics, which are reviewed quarterly.
