With the number of people aged 65 and older expected to more than double by 2050, according to the United Nations, demand for innovative, sustainable, and community-centered care solutions has never been more urgent. On Lok, the pioneering nonprofit behind the Program of All-Inclusive Care for the Elderly (PACE), an alternative to nursing homes, is leading the way with a proven model that empowers seniors to age with dignity, independence, and comprehensive support.

“Simplify Your Life: At Your PACE,” this year’s theme for September’s National PACE Month, perfectly encapsulates the mission of the PACE program, which streamlines and delivers all necessary services, allowing seniors to live comfortably and authentically in their homes and communities.

With dedicated caregivers, transportation, comprehensive medical care, and social opportunities, On Lok PACE has been enhancing seniors’ quality of life since its inception. Today, On Lok PACE serves over 2,100 seniors in San Francisco, Southern Alameda, and Santa Clara counties, while nearly 77,000 frail seniors are served nationally by 171 PACE organizations across 33 states and the District of Columbia.

A Model for the Nation in Senior Care

On Lok’s PACE model has been recognized nationally as the "gold standard" for senior care, offering all Medicare and Medicaid services to seniors aged 55 and older who require nursing home-level care but prefer to remain in their communities.

National Impact in 2023: By the Numbers

  • 95% of Participants Remain at Home: 95% of PACE participants continue to live independently, avoiding the isolation and high costs of nursing home care.
  • 97.5% of Caregivers Recommend PACE: Family caregivers overwhelmingly recommend PACE, highlighting its effectiveness and compassionate approach.

Local Impact in 2023:

  • 1,000,000 Miles Driven: On Lok PACE covered over 1 million miles to transport seniors and deliver essential supplies like food and medicine, ensuring uninterrupted care and support.
  • 2,145 Bay Area Seniors Served: On Lok PACE provided culturally and linguistically appropriate health care ensuring service that respects their diverse backgrounds.
  • 442,500 Meals Served: Last year, On Lok provided 442,500 nutritious meals to PACE participants in their homes and at centers, helping to combat food insecurity among the elderly.
  • 108,000 Medical Appointments Facilitated- Seniors were assisted with 108,000 medical appointments to improve health and quality of life.
  • 308,400 Service Hours in Comforting and Familiar Home Environments: On Lok PACE ensured that seniors had access to timely and necessary supportive home care.

Championing Vulnerable Communities

In addition to these impressive achievements, On Lok continues to address the needs of the most vulnerable seniors. Openhouse + On Lok Community Day Services, the nation's first community-based adult day program designed for and with the LGBTQ+ community, exemplifies this commitment. With participants at high risk of malnutrition, social isolation, and other challenges, this program is a critical resource, offering meals, personal care services, and a safe, supportive environment.

PACE, originating in the Bay Area, is an Innovative senior service program with 171 PACE organizations nationwide. The PACE model has expanded to 33 states and the District of Columbia, serving over 77,000 participants nationwide.

The National PACE Association (NPA) is dedicated to nationally enhancing the efforts of PACE programs, which offer coordinated preventive, primary, acute, and long-term care services. These services enable older adults to continue living in their communities. Care is provided by interdisciplinary teams that include primary care professionals, registered nurses, home care coordinators, personal care attendants, dietitians, social workers, drivers, PACE center managers, and physical, occupational, and recreational therapists.
