Addressing unmet needs that impact mental well-being
Nearly half of all adults in the U.S. experience at least one unmet social need, such as food scarcity, inadequate housing and caregiving challenges. Conventional methods of addressing social determinants of health that impact mental health often require individuals to seek out resources on their own. To reduce the burden on those in need, Lyra is investing in proactive, insights-driven outreach by Resource Coordinators to address member’s unique circumstances, including:
- Assessing social care needs: A new well-being assessment as part of Lyra’s core platform will screen for social determinants of health such as social connection, physical health, financial stressors and caregiving challenges. Based on the assessment, members will receive personalized recommendations for relevant Lyra mental health care and well-being resources such as working with a Lyra Resource Coordinator.
- Dedicated support from start to finish: Lyra Resource Coordinators are trained to offer members personalized and compassionate support on topics including financial worries, food or housing insecurity, caregiving needs, and social inclusion. Based on individual member needs, Resource Coordinators will provide referrals for local and national resources, employer-sponsored benefits, and additional Lyra resources as well as assistance with next steps.
- Proactive, insights-driven outreach: Using third-party insights to identify communities with health equity needs, Resource Coordinators are able to proactively offer support and determine the most effective personalized plan, ensuring equal access to mental health support for all.
