Based out of Innovive Health’s clinical operations in Colorado Springs, the members of the team all have a deep background in the home healthcare sector and experience working with complex behavioral health patients.
The key additions follows the recent expansion of the agency’s clinical operations into Colorado. Innovive Health’s unique home health model focuses on patients who have multiple medical comorbidities with complicating behavioral illness, treating, and educating patients on their holistic well-being. The following appointments were announced today:
Daniel Politano, Executive Director of Colorado Operations
Dan Politano has been appointed as the Executive Director of Innovive Health’s Colorado Operations. Dan is responsible for rapidly growing and scaling the agency’s groundbreaking behavioral health home care model in the Rocky Mountain state. He will manage a growing team of clinicians focused on providing skilled nursing services to patients who have multiple medical comorbidities with complicating behavioral illness. Prior to accepting this position, Politano spent more than a decade overseeing several of Innovive Health’s busiest regional offices in Massachusetts. With a strong record of building and leading clinical teams, Dan's capabilities are well-established. He has a background as a registered nurse at Taunton State Hospital and possesses clinical experience from renowned medical centers like Saint Elizabeth's Medical Center and Caritas Carney Hospital. Dan holds an Associate of Science in Nursing from Caritas Laboure College, and he is currently attending the Southern New Hampshire University BSN program.
Leighann Stonefield, Clinical Director
Leighann Stonefield has joined the growing team as Clinical Director where she will leverage her extensive background in healthcare and nursing to help direct Innovive Health’s Colorado clinical team. Her previous roles include serving as the Director of Clinical Services for Nursing and Therapy Services of Colorado, Director of Nursing for Wellpath, a member of the Nursing Faculty at Trinidad State College and working as an ER Psychiatric Nurse for UCHealth. Leighann holds a compact nursing license as well as being licensed in seven additional states, showcasing her broad expertise and ability to navigate various healthcare systems. She earned an Associate of Applied Science degree in Nursing from Trinidad State College, a Bachelor of Science in Nursing from Grand Canyon State University, and a Master of Science in Leadership in Healthcare Systems, also from Grand Canyon State University.
Nina Britton, Case Manager
Nina Britton has assumed the role of Case Manager in Colorado, bringing with her an impressive 20-year career as a dedicated military nurse – delivering exceptional and patient-centered care. Prior to joining Innovive Health, she served as a Medical Nurse Case Manager with the 10th Special Forces group at Fort Carson. Over her diverse career, she’s worked as a Pre-Certification Nurse/Nurse Consultant, Utilization at the Multi Service Market Organization, and Community Health Nurse at Fort Carson. Nina holds a bachelor’s degree in nursing from the University of Southern Colorado Pueblo and a Master of Arts in Health Administration from Webster University – Greenwood Village.
