COLUMBUS, Ohio--(BUSINESS WIRE)--Molina Healthcare of Ohio (“Molina”) announced today an exclusive partnership in Ohio with Cityblock Health (“Cityblock”) to bring its personalized health care program to 10,000 Ohioans in urban Cincinnati, Cleveland, and Columbus. This partnership will launch in 2022 and result in close to 100 new clinical and non-clinical job opportunities across the three communities in the first year alone.
Cityblock is a national company focusing on providing health care access for Medicaid and lower-income Medicare beneficiaries. The company provides personalized primary and behavioral health care for people who are experiencing complex needs in their health and daily lives. This personalized care includes 24/7 access to multi-modal care delivery, where doctors and other clinical and non-clinical care team members treat patients in their homes, community settings, virtually using telehealth, or at Cityblock hubs like the Molina One-Stop Neighborhood Centers to be opened in Ohio.
Molina and Cityblock’s close partnerships with community-based organizations and their focus on addressing social determinants of health will focus on inequities exacerbated by COVID-19, which has disproportionately impacted vulnerable populations. The goal of this partnership is to improve health outcomes by identifying and eliminating systemic inequities like access to nutritious food and stable housing.
Cityblock’s Community Health Partners, who are hired from the communities in which they serve, will work closely with Molina members in Cincinnati, Cleveland, and Columbus on their individual care plans, and will connect them to local community organizations and care teams to facilitate key needs.
