Eccovia Solutions Rolls Out PRAPARE SDoH Screening Tool on its Care Coordination Platform

SALT LAKE CITY--()--Eccovia Solutions, the leading provider of flexible, cloud-based case management and community care coordination software for health and human services organizations, today announced the infusion of the PRAPARE SDoH (social determinants of health) capability into the ClientTrack platform.

The PRAPARE SDoH screening tool incorporates one of the leading protocols nationwide to measure risk factors that negatively impact at-risk populations. This assessment includes a total of 16 social determinant measures, such as housing stability, transportation, employment status, and social support—and helps community health centers and other providers consider how to treat the person instead of simply reacting to emergencies.

Eccovia Solutions’ ClientTrack Care Coordination Platform brings the disparate elements of care coordination into a single cloud-based system, including support beyond the health system, allowing coordinators to create and manage a client’s care plan from a single comprehensive program. ClientTrack focuses on access and brings data to the forefront, including:

  • Advanced Case Management
  • Comprehensive Care Plan Development
  • Multi-discipline Care Team Management
  • Referral Management
  • Prioritization and Risk Stratification
  • Comprehensive Reporting and Analysis Tools

“Managing social determinants of health is a proven strategy to successfully navigate the shift from volume-based care to value-based care,” said Carl Champagne, CEO of Eccovia Solutions. “The addition of PRAPARE to our Community Care Coordination Platform is a direct response to what our clients have been telling us they need to better serve their clients."

SDoH, as measured through standards created by the National Association of Community Health Centers (NACHC), rely on 15 core questions and five supplemental questions to measure a wide range of life quality issues impacting a patient’s health and well-being. These include such factors as food and housing insecurity to healthy behaviors and income. The NACHC screening, known as Protocol for Responding to and Assessing Patients' Assets, Risks, and Experiences (PRAPARE), is one of the leading SDoH tools nationwide, according to the American Academy of Family Physicians.*

As more and more social and health agencies switch to value-based care (results) from the old fee-for-service model (volume), the ability for a data-driven overview of all patient risk factors is key to controlling costs and realizing better outcomes. More and more, funding decisions rely on SDoH solutions.
