INDIANAPOLIS--(BUSINESS WIRE)--Oak Street Health, a network of health centers providing primary care for Medicare patients, and leading health and well-being company Humana Inc. (NYSE: HUM) are expanding the availability of coordinated, patient-centered care in Indiana. Through a value-based care network agreement, Oak Street Health’s newest Indianapolis-area location serves Humana Medicare Advantage members in an effort to help them achieve better health.
The agreement aims to offer quality, cost-effective care and enhance both the patient experience and health outcomes in the city. The new Indianapolis location – at 2240 East 53rd Street (Suite B-1), in the Glendale neighborhood on the city’s north side – is in-network for Humana Medicare Advantage Health Maintenance Organization (HMO) and Preferred Provider Organization (PPO) health plan members in Indiana. Humana’s Medicare Advantage plans are the only Medicare Advantage plans accepted at the center, which also accepts original Medicare.
Opened this month, the Glendale center is Oak Street Health’s fourth in metro Indianapolis, with locations on the south side of the city (in University Heights at 4200 S. East Street); on the east side (in Irvington at Irvington Plaza, 6401 E. Washington Street); and west of the city in Speedway, Indiana (5926 Crawfordsville Road, Unit B). With the addition of the Glendale location, Oak Street Health has seven Indiana centers, including locations in Fort Wayne, Gary, and Hammond.
