CLEVELAND & LOUISVILLE, Ky.--(BUSINESS WIRE)--Cleveland Clinic and Humana Inc. (NYSE: HUM) announced today added benefits to, and broadened availability of, their co-branded, $0 premium Medicare Advantage Health Maintenance Organization (HMO) health plan for 2019. The enhanced Humana Cleveland Clinic Preferred HMO Medicare Plan – which offers patient-centered, affordable access to expert doctors, nurses and facilities – will be available to residents of five northeast Ohio counties.
Combining Humana’s Medicare Advantage experience with Cleveland Clinic’s clinical expertise, the Humana Cleveland Clinic Preferred HMO Medicare Plan is designed to help people with Medicare achieve better health through a more personalized, coordinated, and connected member experience. The plan was launched last year for people with Medicare in Cuyahoga County, and will have expanded availability in four additional counties – Lake, Lorain, Medina, and Summit – during this year’s Medicare Advantage and Prescription Drug Plan annual enrollment period (Oct. 15 to Dec. 7 for plan coverage starting Jan. 1, 2019).
Humana and Cleveland Clinic also offer a second co-branded health care plan, a Medicare Advantage Dual-Eligible Special Needs (D-SNP) plan for individuals in Cuyahoga County covered by both Medicare and Medicaid. Both the D-SNP and HMO plans offer affordability and accessibility with a $0 monthly plan premium; a $0 primary care provider office visit copay; and a $0 copay for a 30-day supply of Tier-1 prescription drugs. In addition, neither plan requires a referral to see in-network specialists.
In 2019, the two plans will be enhanced through the addition of new plan benefits, including:
- A personal emergency alert system “help button” – a lightweight device that can be worn around one’s neck or wrist, or on a belt – which can be activated 24/7 to call for assistance with urgent medical needs. The system also is designed to activate when detecting a patient fall.
- Coverage of on-demand, live telemedicine visits through Cleveland Clinic Express Care® Online, which offers secure, cost-effective care for many common illnesses and health problems.
- New to the Humana Cleveland Clinic Preferred HMO Medicare Plan will be a transportation benefit to provide members with rides to plan-approved locations for non-emergency medical purposes, such as for medical appointments and dialysis, and to fitness centers (the transportation benefit already is a D-SNP plan feature).
Members of both plans will have access to Cleveland Clinic health system’s more than 3,500 full-time salaried physicians and researchers, representing 140 medical specialties and subspecialties, as well as independent physicians who are part of the Cleveland Clinic Quality Alliance. The Cleveland Clinic health system includes the main campus hospital and facilities near downtown Cleveland, 10 regional hospitals and more than 150 outpatient locations in northeastern Ohio - including 18 full-service Family Health Centers and three Health and Wellness Centers.
In addition, members can take advantage of services provided by Cleveland Clinic’s navigator team, a support unit that provides patient guidance and assistance in scheduling appointments and coordinating care.

- Access to proactive health screenings and programs that are focused on preventing illness;
- Improved care for people living with chronic conditions with a focus on avoiding health complications;
- Leveraging technologies, such as data analytics, that connect physicians and help them work as a team to coordinate care around the patient; and
- Reimbursement to physicians linked to the health outcomes of their patients rather than based solely on the quantity of services they provide (fee-for-service).
