Geisinger’s initial work with AVIA will focus on innovative solutions to address value-based care and community health needs. Up first is the Medicaid Transformation Project, a collaborative effort with peer health systems to find financially-sustainable solutions that deliver high-quality care for Medicaid and other vulnerable populations. “Improving the health and care of our Medicaid population is one of our most complex community challenges,” said Dr. Murphy. “We’re excited and eager to work with AVIA and peers who also want to prioritize and accelerate solutions for vulnerable populations.”
Geisinger has launched successful community health innovation initiatives, including Springboard Healthy Scranton, a first-of-its-kind program designed to improve an entire community’s health. The program develops, tests, and implements projects to coordinate community resources, eliminate hunger, prevent chronic diseases, and unleash the power of genomic medicine. The program will share lessons learned and proven solutions both nationally and globally. As part of its Springboard Health Initiative, Geisinger also launched the Fresh Food Farmacy, which addresses health challenges faced by patients with diabetes and food insecurity. The goal is to improve access to both healthy food and nutritional knowledge. The target population has high rates of obesity, pre-diabetes, and diabetes, and early results show significant improvements in blood sugar control, weight, and overall health management.
