SALT LAKE CITY--(BUSINESS WIRE)--Collective Medical, delivering the nation’s largest and most effective network for care collaboration, today announced a partnership with and an endorsement by the Texas Hospital Association (THA). Through the partnership, care teams in Texas will have access to the Collective network, providing the information needed to identify and support complex patient populations. The partnership also supports THA’s recent voluntary guidelines for hospital emergency department (ED) prescribers of opioids by arming care teams with real-time, relevant patient information at the point of care.
Collective is currently partnered with more than a dozen state hospital associations. Collective’s real-time, risk-adjusted event notification and care collaboration platform serves all points of care and supports care teams across emergent, inpatient, post-acute, mental and behavioral, and ambulatory settings, as well as stakeholders in ACOs and health plans.

The Collective network has improved care collaboration in healthcare organizations across the country. As an example, CHI St. Anthony Hospital, a critical access hospital located in Pendleton, Oregon, was able to reduce unnecessary ED visits from identified frequent ED users from 17 percent of overall visits to nine percent within six months of implementing the Collective platform. Within one year, the hospital reduced narcotic prepack prescriptions coming out of the ED by 60 percent and realized hospital cost savings of $200,000. As of April, 2018, after 18 months of using Collective’s platform, the hospital has reduced unnecessary ED visits by identified frequent ED users to just 4.25 percent.
Columbia Medical Associates, a multi-specialty group practice participating as an ACO in the Medicare Shared Savings Program, saw a 15 percent decrease in ED utilization which amounted to an approximate $6.5 million decrease in total cost of care from 2016 to 2017 after implementing the Collective platform. As a result of its program, the group’s patient satisfaction scores also rose to the highest in the State of Washington.
