Collective Medical Partners with the Florida Hospital Association

Partnership to target reductions in avoidable hospital readmissions, further enable state-wide opioid crisis mitigation efforts, and better manage preventable emergency department utilization

SALT LAKE CITY--()--Collective Medical, delivering the nation’s largest and most effective network for care collaboration, today announced a partnership with the Florida Hospital Association (FHA).

The partnership facilitates FHA member hospitals’ access to the Collective network and EDie solution, a real-time, risk-adjusted event notification and care collaboration platform. By helping FHA members identify and support high-risk patients in need and facilitate collaboration of providers across care settings, the partnership aims to reduce avoidable readmissions and further enable state-wide efforts to address the opioid epidemic, and better manage complex patients who frequently seek care in emergency departments (ED).

Collective is currently partnered with more than a dozen state hospital associations. Independently or in collaboration with public and private Health Information Exchanges (HIEs), Collective supports care teams across emergent, inpatient, post-acute, mental and behavioral, and ambulatory settings, as well as stakeholders in accountable care organizations and health plans.

In Washington, Collective’s first state-wide initiative, a Brookings Institution review of Medicaid patients who visited emergency rooms found that together with the Collective network and EDie product, the state saved $34 million in emergency department costs and ED visits declined 9.9 percent in its first year of use in 2013. Likewise, care teams across the state have reduced opioid prescriptions coming out of the ED by 24 percent since the program’s inception.
