In partnership with one of the largest community-based health systems in Florida, Perficient will present “Improving Transitions of Care for High-Risk Patients.” During this presentation, the team will showcase how, through the use of IBM Watson technology, the healthcare system innovated with natural language processing to manage its Hospital Readmissions Risk and address MS-DRG bundled payments.
