Collective Medical and Consonus Healthcare Announce Partnership to Improve Post-Acute Transitions of Care

Partnership makes PreManage platform available to more than 300 skilled nursing facilities; enables streamlined care coordination and more effective transitions of care

This partnership will help make Collective Medical’s care collaboration platform, PreManage, available to more than 300 Consonus skilled nursing facilities (SNFs)—allowing care teams to collaborate with hospitals and health systems to streamline care transitions in post-acute care settings.

The partnership will deliver enhanced value by allowing these post-acute care providers the opportunity to demonstrate better outcomes—particularly related to preventing readmissions—and improving care management collaboration with hospitals. Consonus-sponsored SNFs will receive intelligent alerts that provide insights into a patient’s readmission risk, specific care guidelines and plans, utilization patterns and advance directives, as well as notifications on patients with a history of being violent or a security risk.

Collective Medical’s real-time, intelligent event notification and care collaboration platform—PreManage—serves all points across the continuum of care, including emergent, in-patient, post-acute, mental and behavioral, and ambulatory settings, as well as stakeholders in ACOs and health plans.

Inadequate care transitions—particularly those between care settings—can cause significant harm to patients and present financial risk to providers. A 2010 report found that 23.5 percent of Medicare beneficiaries discharged from a hospital to a SNF were readmitted within 30 days. Broadly, The Centers for Medicare and Medicaid Services states that nearly 20 percent of Medicare patients discharged from a hospital are readmitted within 30 days, at a cost of more than $26 billion every year.
