NEW YORK--(BUSINESS WIRE)--TripleCare, a national provider of telemedicine-based healthcare services to skilled nursing facilities (SNFs), announced today that the Company has entered into an agreement to provide virtual telemedicine services to three Wisconsin Illinois Senior Housing Inc. (WISH) skilled nursing facilities (SNFs) managed by Lakewood, Colo.-based Carriage Healthcare Companies, Inc. (CHC).
Effective early 2018, TripleCare will introduce services to one SNF in Illinois, and two in Wisconsin, marking the Company’s entry into that state. The three SNF facilities and locations are:
- Montello Care Center, a 50-bed facility located at 251 Forest Lane, Montello, WI 53949, 608-297-2153
- Edgerton Care Center, a 61-bed facility located at 313 Stoughton Road, Edgerton, WI 53534, 608-884-1330
- Fair Oaks Health Care Center, a 51-bed facility located at 471 W. Terra Cotta Avenue, Crystal Lake, IL 60014, 815-455-0550
Each location is owned by WISH and managed by CHC, a multi-state operator of SNFs, assisted living facilities, independent living facilities and home health agencies, primarily in rural communities.
TripleCare’s virtual healthcare services employ telemedicine protocols during times when physicians are not on site, such as overnight, weekends and holidays. The Company’s network of highly trained physicians perform virtual bedside visits, treating patients in place, using advanced technology, coupled with assistance from on-site nurses at each of the SNFs. TripleCare’s virtual services make it possible for its physicians to differentiate patients from those that can be treated in place versus ones requiring a hospital transfer.
