Eccovia Solutions™ and Community Health Partnership Join Forces to Bring Integrated Care Coordination to Members in Southern Colorado

SALT LAKE CITY--()--Eccovia Solutions announced today that Community Health Partnership (CHP), a coalition of more than 25 provider organizations in southern Colorado, has launched ClientTrack™ – Eccovia Solutions’ industry-leading community care coordination platform – to help provide integrated care for members of its program, Community Care of Central Colorado. Community Care coordinates care and delivery of services for Health First Colorado (Colorado’s Medicaid Program).

Coordinating care across Community Care’s population requires use of a proven platform specifically designed for this purpose, which ClientTrack is able to provide. The platform will enable CHP to move forward their mission of improving access and controlling cost for the Health First program. Improving access to healthcare and associated resources is fundamental to providing the highest quality of care.

“The goal of CHP is to improve the health and wellbeing of the Pikes Peak community through collaboration and whole-person care coordination,” said Jim Calanni, CHP’s Chief Technology Officer. “Using the Fast Healthcare Interoperability Resources (FHIR) data exchange standards, ClientTrack integrates with CHP’s robust data warehouse, ALLURData, and the CORHIO health information exchange. This integration enables 25 providers to collaborate and share encounter data in order to refer people for assistance and care from the most appropriate agency for their unique needs, and measure and report on their outcomes.”

For more than 20 years, ClientTrack Case Management has played a critical role in helping community and government organizations across the country improve administrative operations to more effectively serve individuals in the community. Because ClientTrack can support a variety of government, health and social agencies at the same time, CHP providers, members and community partners have the potential to improve care coordination from a single, comprehensive view, reducing costs and improving outcomes.
