WOODLAND HILLS, Calif.--(BUSINESS WIRE)--Anthem Blue Cross (Anthem) today announced the launch of a new population health system connecting patient information among all seven original Vivity hospital systems. These connections give providers access to robust information about patients, including claims history and clinical documents such as discharge summaries across the continuum of care.
Vivity launched more than two years ago as a first-in-the-nation product partnership between a health plan and seven like-minded hospital systems to provide a virtually integrated health care system experience that delivers high-quality, cost-effective care and coverage. The seven hospitals systems – all with hospitals ranked in the top 30 in Los Angeles County by U.S. News & World Report – are Cedars-Sinai, Good Samaritan Hospital, Huntington Hospital, MemorialCare Health System, PIH Health, Torrance Memorial Medical Center and UCLA Health.
Through a separate company within Anthem, Vivity has launched a population health management platform, called Vivity Provider Care Management Solution, which leverages clinical and claims data across the seven hospital systems and creates a single, comprehensive record for each Vivity member. This allows for greater transparency into a patient’s medical history and promotes more effective coordination of care across all Vivity partners. The tool also identifies high-risk members who are at increased likelihood for hospital readmission and flags members with gaps in care, such as missed diabetic testing or colorectal cancer screening. A dedicated care team works with each hospital system to leverage the insights and data delivered through the platform and apply it to care coordination activities across Vivity.

The Vivity Provider Care Management Solution platform also provides analytics, making it easier for providers to share best practices and continuously improve quality across the system. For example, the tool can identify one or two day admissions across hospitals to facilitate a team discussion on lowering avoidable admission rates. Vivity leadership can also view the quality analytics to identify underperforming areas and build strategies to improve.

For members, the tool allows a new level of convenient care – for example, a member can have lab work taken at one Vivity hospital that can be reviewed by a doctor at another Vivity partner’s medical group.
