SAN DIEGO--(BUSINESS WIRE)--MD Revolution, the leader in digital chronic care management (CCM), is partnering with Reliance Medical Group, a 40 provider primary care organization, to improve patient engagement, enhance the patient-provider relationship, speed clinical decision-making, and increase revenue. With the Medicare compliant RevUp For CCM (RevUp) Solution, physician practices can outsource CCM and population health to MD Revolution with minimal disruption to practice operations.
RevUp integrates with the practice’s electronic health record (EHR) to create the patient care plan and make it available anytime, anywhere securely. Using this care plan, MD Revolution’s clinicians act as a trusted extension of the physician to help patients meet their health improvement goals through earlier detection of health issues, medication compliance and nutrition and fitness advice. RevUp includes a smart phone application and Internet portal with support for multiple modalities including patient monitoring devices, fitness trackers, paper, and telephone to engage patients and securely gather patient-provided data. This data is integrated with the EHR to provide a comprehensive view of the patient’s health in between office visits.
“We are excited to partner with a group that see digital technologies as central to patient engagement now and in the future,” said Samir Damani, MD, a practicing cardiologist and MD Revolution’s founder and CEO. “RevUp provides insight into patient health in between office visits in a high-touch, low-cost way that has never been possible before. As healthcare shifts to a value-based model, Reliance will be well-positioned to make this leap and fundamentally improve how patients view their health care experience.”
With millions of Americans suffering from chronic, preventable conditions, CCM is becoming increasingly important for physicians and policymakers. Under new Medicare rules, physicians can provide 20 minutes of remote consultations each month for their chronic care patients. However, most physician practices lack the technical expertise and infrastructure to implement these regulations. MD Revolution provides the digital tools and services that practices need to identify eligible patients, engage them remotely, and bill appropriately.
Based in Pleasantville, New Jersey, the Reliance Medical Group cares for more than 50,000 people throughout the state. With RevUp, they hope to provide better care for more patients.
“Reliance believes healthcare is a right, not a privilege, and we are proud to bring this chronic care management program to our offices,” said Jon Regis, MD, president and CEO of Reliance Medical Group. “Providing this top-notch care for elderly patients is both rewarding and severely needed. Reliance has been on the front lines, improving health care in these areas. We are excited to see the better outcomes CCM will bring to our population.”
“Programs like RevUp are the future of healthcare,” said Vince Papaccio, vice president of Reliance Medical Group. “This minute-by-minute connectedness between the care team and patients supports 24/7 wellbeing. We are also excited that this program fits our patient-centered medical home model to improve quality while reducing the cost of care.”
