SAN ANTONIO--(BUSINESS WIRE)--Improved quality of life and more coordinated health care services for employers and individuals in San Antonio and South Texas is the goal behind a new Accountable Care agreement between Partners in Primary Care and Humana Inc. (NYSE: HUM).
Locally based employers already with Humana health insurance and those businesses seeking alternative choices for health insurance for their employees will find that accountable care agreements reflect a shift away from the current episodic health care model to a value-based model that is designed to lower costs and emphasizes quality and patient-care experiences. Members with HumanaOne individual insurance, both on the marketplace health insurance exchange and off the marketplace exchange, may also seek care through this accountable care arrangement.

Under the agreement, primary care services provided by Partners in Primary Care for Humana commercial members will be available at 10 medical clinics in the surrounding area, including San Antonio, Floresville, Seguin and New Braunfels. Partners in Primary Care already provides primary care services for Humana’s Medicare Advantage members.

The agreement includes value-based incentives tied to performance improvement and supports successful population health management and a world-class patient experience for Humana members and Partners in Primary Care patients in South Texas. The arrangement offers more personalized care and a broad range of population health tools like predictive analytics, chronic care, disease management, care coordination and wellness programs.
The value-based incentive arrangement emphasizes quality outcomes and leverages standard measures defined by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS) such as emergency room visits per 1,000, breast cancer screenings, avoidance of antibiotic treatment in adults with acute bronchitis, and diabetes HbA1c testing.
In addition, physician practices have the opportunity to receive an annual wellness bonus for improvements in their collective Humana patient population.
