(NYSE: HUM), one of the country’s leading health and well-being companies, announced a new multi-year Accountable Care agreement today for key metropolitan areas across Ohio, including Cleveland, Cincinnati, Youngstown and Toledo.
Accountable care is a fast-growing model for health care service delivery and physician payment. The model focuses on creating health in addition to treating sickness, utilizes the primary care physician to coordinate patient care, offers treatment options based on proven standards of care, and better controls costs through integrated care and reimbursement based on adherence to best practice standards of care.
The new Humana/HealthSpan accountable care agreement emphasizes quality outcomes and care coordination with a focus on measures defined by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS) like diabetes care and treatment, breast cancer screenings, colorectal cancer screenings and high-risk medication. The partnership will also offer more personalized care and a broad range of population management tools like predictive analytics, chronic disease management and wellness programs.
