Health Choice Preferred and Phoenix Children’s Care Network Partner to Coordinate Patient Care and Improve Outcomes

PHOENIX--()--Health Choice, a managed care solutions organization and division of IASIS Healthcare LLC, and Phoenix Children’s Care Network, LLC (PCCN) have announced an accountable care alliance focused on providing personalized, high-quality pediatric care for PCCN patients and members of Health Choice Preferred, a non-Medicare accountable care organization.

Under the new agreement, PCCN will serve as a leading provider of pediatric care for Health Choice Preferred ACO members. The partnership initially will focus on providing quality care and improved outcomes for enrollees in Health Choice Arizona’s acute Arizona Health Care Cost Containment System (AHCCCS) Medicaid plan through an accountable care collaboration designed to improve overall cost savings for the state. PCCN also will serve Health Choice Preferred ACO’s 15,000+ members in Arizona and will benefit the many families with employer-sponsored and commercial-based insurance coverage that have already selected the plan.

This accountable care alliance will focus on quality care delivery, enhanced information reporting and sharing, and provider collaboration. Patients and members will benefit from having access to a network of pediatric primary care providers and specialists committed to offering coordinated care and improved outcomes while effectively managing costs.

“We are proud of this strategic collaboration with Phoenix Children’s Care Network, particularly given its established reputation for offering world-class quality healthcare to children and their families,” says Mike Uchrin, Chief Executive Officer for Health Choice. “Working together, we know we can achieve improved outcomes and better value for our members and patients.”

Anne Garrett, MD, board chair of Phoenix Children’s Care Network, agrees.

“Phoenix Children’s Care Network is committed to providing the highest quality care and consistently improving upon our patients’ experience,” she says. “Through our collaboration with Health Choice Preferred ACO, our patients will have access to the benefits of comprehensive care teams, improved data collaboration, and an enhanced level of care.”

The arrangement leverages quality care measures defined by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS). Population management tools employed by Health Choice Preferred ACO and PCCN will include predictive analytics, disease management and wellness programs.
