The release includes new features and functionality that further expand Jiva’s already-robust capabilities to support successful accountable care and value-based health delivery models.

Jiva 5.6, which is scheduled for general availability this summer, offers a comprehensive array of enhancements to help healthcare organizations deliver sophisticated population health management programs that efficiently enhance the value of health care services. This major release features a streamlined user interface that simplifies gathering of patient assessment data, new dashboards and registries, enhanced care transition capabilities, and streamlined connectivity with clinical data sources. Additionally, several technological infrastructure enhancements will enable Jiva to consume increasingly diverse and larger volumes of data, a capability that will become critical as more stakeholders use Jiva to drive care collaboration.

According to Christopher Mathews MD, senior vice president and chief medical officer for ZeOmega, Jiva’s new enhancements will address a number of CMS and other regulatory requirements. “Jiva 5.6 makes it easier than ever to focus on and improve performance for a number of Medicare Advantage STAR ratings and ACO performance measures,” says Dr. Mathews. “Additionally, Jiva 5.6 also includes medication review and management capabilities. These capabilities represent the first phase of a broader ZeOmega initiative to launch a completely integrated, CMS-compliant Medication Therapy Management solution in 2015.”

Jiva 5.6’s latest enhancements include:

  • A set of provider dashboards that allow ZeOmega clients to examine provider utilization, clinical quality, and service quality. It also helps monitor overall performance and enables clients to direct providers on areas of improvement.
  • An enhanced population healthcare management registry that is designed to, among other things, streamline and automate patient identification and stratification processes according to CMS qualification guidelines, improve gaps-in-care functionality, and identify and monitor high-risk admission/readmission patients based on proprietary algorithms within a powerful inpatient census view.
  • Upgrades to the integration architecture that enable seamless connectivity with diverse clinical data sources.
  • Upgrades to Jiva’s Sentinel Rules Engine to enable rules-based predictive, retrospective and prescriptive analytics using EMR, self-reported and claims data. This will streamline clinical performance measurement and management across the care continuum.
