Advocate Health Network Goes Live with Caradigm Knowledge Hub and Caradigm Care Management

Improves Patient Outcomes and Care Efficiency for its High-Risk Patient Populations

BELLEVUE, Wash.--()--Caradigm®, the leader in enterprise population health, today announced that Advocate Health Network (AHN), the Accountable Care Organization (ACO) associated with St Joseph’s/Candler Health System, has implemented Caradigm Care Coordination solutions to support clinical decisions at the point-of-care.

AHN is a participant in the Shared Savings Program by the Centers for Medicare & Medicaid Services. To better prepare for value-based care delivery and the needs of its patients, AHN turned to Caradigm for care coordination that would span both inpatient and outpatient settings, as well as combine its claims data and multiple electronic medical record systems.

AHN is using Caradigm Knowledge Hub to obtain patient-focused data and insights from the Caradigm Intelligence Platform and provide physicians with relevant analytics at the point-of-care. Patient data is shown within clinician workflows alongside the electronic medical record (EMR) or other patient record system. With rapid access to more complete patient data within the EMR, AHN clinicians gain additional insight about the actions needed to provide high-quality care. AHN plans to connect all participating physician offices to Caradigm Knowledge Hub.

The Care Coordination component of Caradigm enables AHN multi-disciplinary care teams to share information and streamline workflows as patients move from acute to post-acute settings. The Caradigm Intelligence Platform powers the program for AHN, aggregating and normalizing clinical, operational and financial data from systems across the community — including EMRs, billing systems, payers, pharmacy systems, labs and health information exchanges. With Caradigm, AHN intends to integrate evidence-based clinical guidelines into patient-centric care plans to facilitate improved patient outcomes and care efficiency for its high-risk patient population.
