Primaria Health Expands its Network of Primary Care Physicians to Become the Largest in Central Indiana and Forms ACO

Provider Partners Are Supported in Keeping Their Patients Healthier

INDIANAPOLIS--()--Primaria Health, a primary care management services organization, has expanded its network of primary care physicians to become the largest in Central Indiana and formed a Medicare Accountable Care Organization (ACO) to expand its portfolio of value-based reimbursement contracts offered to physicians. In addition to contract access, Primaria provides physicians with data, clinical care models, and resources to enable them to improve patient outcomes and patient satisfaction, lower the total cost of care, and perform successfully in the new value-based reimbursement environment.

In the past year, Primaria has grown the number of patients under its model to 130,000 and added more independent physicians to its network, which now totals more than 375 primary care providers. The network recently established an Accountable Care Organization (ACO) which now allows providers to participate in value-based reimbursement contracts for Medicare in addition to commercial populations. In addition to the ACO, Primaria offers physicians commercial and Medicare Advantage value-based contracts with Anthem Blue Cross Blue Shield and manages contracts for another five large national and local payors under its partnership with Community Health Network. Primaria will manage over one billion dollars in medical costs in 2017.

Primaria’s partner providers gain access to proven processes and technology that have demonstrated significant clinical improvements in patients dealing with diabetes, chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF), coronary heart disease and other complicated health conditions. These resources include sophisticated data analysis to identify patients at most risk for a variety of health conditions and complications, as well as those living with chronic conditions; education and support to help patients learn how to manage their conditions, stay on their medications, remove barriers to improving their health, and/or successfully transition home from the hospital; care coordination to keep primary care and specialist physicians aware of care received by the patients they share; and behavioral health management.
