BATON ROUGE, La.--(BUSINESS WIRE)--eQHealth Solutions has been selected by Vigilance Health to provide cloud-based population health management technology to support chronic care management (CCM) and care coordination initiatives in 51 out of 58 counties in the state of California. Intent on helping their customers make the transition to value-based care, improving patient outcomes, reducing costs and gaining efficiencies, Vigilance will provide consulting services, leverage eQSuite technology, and provide the personnel to fulfill the requirements outlined in CMS’s CPT Code 99490 as well as targeted care to high-risk populations. More specifically, eQHealth’s population health management technology platform will serve as the foundation for CCM and care coordination activities workflow, the data warehouse for clinical and claims data, provide analytics driving the business decisions, and tracking, documentation and audit trail reporting - all the necessary components to meet CPT code 99490 billing requirements.
Beginning January 2015, the Centers for Medicaid and Medicare (CMS) recognized CCM as a critical component of primary care for Medicare patients, especially those with two or more chronic conditions. By establishing CPT code 99490, CMS provided a method for healthcare providers to be reimbursed for monthly care given to beneficiaries with chronic conditions to help improve outcomes and control costs. However, just last month, CMS reported that submitted claims for CPT code 99490 in 2015 were very low, less than 2% percent of the total eligible population. It is widely believed that chronic care management and care coordination are foundational elements in the shift to value-based care and in the 2017 Proposed Revisions to the Provider Fee Schedule, CMS is addressing provider barriers with the goal of increasing participation in this primary care initiative.
