WESTBOROUGH, Mass.--(BUSINESS WIRE)--eClinicalWorks®, a leader in ambulatory healthcare IT solutions, announces today that Wilderness Health, a non-profit collaborative of independent providers in Northeast Minnesota and Northwest Wisconsin that is recognized as an Integrated Health Partnership (Minnesota Medicaid ACO), has chosen eClinicalWorks CCMR for population health. eClinicalWorks CCMR will assist Wilderness Health in coordinating care across the continuum, including nine health systems, as well as evaluating population health and quality improvement activities.
“Wilderness Health is focused on advancing patient and community health outcomes, improving the patient experience and lowering costs,” said Cassandra Beardsley, executive director of Wilderness Health. “Our rural setting creates both challenges and opportunities as we look to identify potential gaps in care and patients that can benefit from care coordination. Partnering with eClinicalWorks will aid us in doing just that as well as tie together several disparate systems, enabling better care for our patients.”
eClinicalWorks CCMR will provide Wilderness Health with visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. This includes nine health systems with multiple electronic health records (EHR) systems. A vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payer claim feeds, including CMS, to provide cost and utilization for services delivered to patients.
“Wilderness Health covers a mostly rural region, with significant distance between patients and providers,” said Girish Navani, CEO and co-founder of eClinicalWorks. “eClinicalWorks provides the organization with technology for a more comprehensive approach to patient care as well as deeper insight into its patient population to help the organization discover actionable opportunities for improved outcomes, while connecting providers.”
Included in CCMR, eClinicalWorks Population Health Management Analytics uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage population health effectively. Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.
