BHN will also utilize eClinicalWorks CCMR for population health management. Layering eClinicalWorks CCMR provides visibility into a patient’s care across all settings, facilitates smooth transitions of care, engages patients and providers in preventative care, improves outcomes among populations and reduces costs. Using a population health platform like this allows for a window into the multiple EHR systems used by affiliated physicians, which is useful for ACO measures as well as a providing data back to the practices.
