ROSEVILLE, Calif.--(BUSINESS WIRE)--Intel-GE Care Innovations™, a joint venture between Intel Corporation and GE Healthcare, today announced a partnership with Front Porch communities and the Front Porch Center for Innovation and Wellbeing to measure the benefits of leveraging remote patient monitoring (RPM) with senior living residents.
Residents of Kingsley Manor in Los Angeles and Claremont Manor in Claremont, California, will use Health Harmony, Care Innovations’ RPM platform that simplifies RPM by providing intuitive and easy-to-use technology that enables individuals, clinicians, care providers, and family caregivers to collaborate for better care in the home – wherever home may be – and within normal daily routines. Initially, Health Harmony will be available to a group of Front Porch residents who may require a higher level of care and monitoring due to poor chronic condition management or following a return to a senior living community after a discharge from a skilled nursing facility.
“Health Harmony gives Front Porch a way to extend the care continuum directly to their residents while also supporting the independence they strive to deliver every day,” said Sean Slovenski, CEO of Care Innovations. “As the aging population continues to grow, it’s important that senior living communities are part of efforts to help people live healthier lives at a more sustainable cost. Front Porch is a forward-looking organization taking a fresh look at how technology can help them with resident well-being, which we know is one of their major priorities.”
Residents of Kingsley Manor and Claremont Manor who use Health Harmony will conduct daily health sessions to capture vital signs such as blood pressure, weight, oxygen levels, and glucose levels, subjective health assessments, and other information needed for caregivers to manage care plans, promote medication adherence, and remain on top of patients’ changing health. Residents also have access to a video chat platform through Health Harmony that enables a caregiver and the resident to virtually connect with one another to discuss conditions and assess a resident’s environment.
“Front Porch and Care Innovations share a common vision of technology-enabled care to help the aging population live well and remain healthy wherever they call home,” said Kari Olson, president of the Front Porch Center for Innovation and Wellbeing. “Our partnership with Care Innovations opens opportunities for our communities to become more deeply involved in helping residents manage their health with new models of care in a way that meets their specific needs.”
RPM is a home health care strategy that allows a resident or patient to take blood pressure, weight, oxygen, glucose and other vitals readings with little or no help from a caregiver or nurse, and transmit these data to the provider who can proactively and appropriately respond to alerts before an emergency arises. According to the Center for Technology and Aging, “RPM technologies provide essential support for the coordination of care, behavior change (of providers as well as patients), and evidence-based decision support for patients.”
