MD Revolution Provides Turnkey Solution for Chronic Care Management

Proven model increases patient engagement and enables physicians to take advantage of new Medicare payments for chronic care management in 2015.

SAN DIEGO--()--MD Revolution has developed a user-friendly system to support physicians providing chronic care management (CCM) services for Medicare patients. Called RevUp, this proven platform combines digital monitoring devices with personalized intensive coaching, data from lab tests and machine learning to improve patient engagement.

In recognition of the avalanche of chronic diseases overcoming the U.S. and the services that physicians already provide, the Centers for Medicare & Medicaid Services (CMS) will pay for chronic care management as part of their 2015 Physician Fee Schedule (PFS). As a result, CMS has established a new code to pay physicians $42 per month for non-face-to-face management of patients with two or more chronic conditions.

  • 70 percent of patients experienced weight loss and were able to change their body composition profile, losing dangerous visceral fat;
  • 80 percent improved their cardiorespiratory fitness;
  • More than 75 percent remained engaged with the program.

MD Revolution has already inked a partnership with Elevate Seniors, who will utilize the RevUp platform to provide practices with comprehensive care management, and Health-e-Link, a next generation SaaS provider of cloud-based electronic health record systems, who will offer RevUp to all qualifying patients in Health-e-Link’s system. The company is working with other EHR vendors as a service provider for CCM and with a number of large health systems.
