SAN DIEGO--(BUSINESS WIRE)--MD Revolution has developed a user-friendly system to support physicians providing chronic care management (CCM) services for Medicare patients. Called RevUp, this proven platform combines digital monitoring devices with personalized intensive coaching, data from lab tests and machine learning to improve patient engagement.

As a result, CMS has established a new code to pay physicians $42 per month for non-face-to-face management of patients with two or more chronic conditions.
