SCAN Health Plan Arizona Announces 2015 Benefits for Maricopa and Pima Counties

Optum Medical Network Joins SCAN Provider Network Effective Jan. 1, 2015

PHOENIX--()--SCAN Health Plan Arizona announced today its 2015 Medicare Advantage Prescription Drug plan (MAPD) benefits for Maricopa and Pima counties. The SCAN Classic (HMO) plan will continue to include comprehensive preventive and hospital care plus complete prescription drug coverage – with no monthly plan premiums and no deductibles.

In addition, Optum Medical Network will join the SCAN Health Plan provider network effective January 1, 2015. Formerly known as Lifeprint, Optum Medical Network includes a large network of primary care physicians, specialists and hospitals throughout the greater Phoenix area. As a result, SCAN members in Maricopa County will have access to an expanded network of primary care and specialty physicians. Members will also continue to have access to DaVita HealthCare Partners Medical Group.

The SCAN Classic benefits in Maricopa and Pima counties include:

  • No monthly premium.
  • No co-payments for primary care office visits.
  • No co-payments for annual physical exams.
  • No co-payments for screenings for breast, prostate and colorectal cancers.
  • No charge for diabetic supplies (lancets, test strips, monitor) and diabetic self-management training.
  • Free access to Healthways SilverSneakers Fitness Program.

Seniors and others on Medicare can enroll in SCAN Classic during the annual enrollment period of Oct. 15 – Dec. 7, 2014. During this time Medicare beneficiaries may choose to switch their health plan or move from traditional Medicare to a Medicare Advantage plan (or move back to Original Medicare) with their new elections taking effect January 1, 2015.
