The technology enhances Sage’s ability to help providers successfully adapt to the emerging outcomes-based reimbursement models, holding down costs while also improving care.
Navigate is a cloud-based population health and behavioral change platform that enables providers to integrate patient data from multiple clinical and financial systems, and to explore the data according to any combination of variables. Using both industry-standard and customized risk assessment models, providers can identify low-risk, rising-risk and high-risk patients.
Navigate then generates goal-based care plans for individual patients, and provides intuitive dashboards and workflow tools to care managers. These help care managers engage patients in better self-management and close gaps in care, ensuring that patients get the right test and treatments, when they need them. This both improves patient outcomes and, ultimately, bends the cost curve.

For example, Navigate distinguishes between a patient with diabetes who is at low risk (someone who is moderately active with a controlled weight) from someone who is at high or rising risk (someone with additional complications such as high blood pressure or who has gone to the emergency room within the past six months.)
Both groups need A1c blood tests every six months to monitor how well their diabetes is being controlled, but their care plans would involve different intervention intensity. The low-risk individual might receive an email reminder if s/he hasn’t had the test in more than six months. The higher-risk individual might get a secure message and a phone call or even a visit from a care manager. Navigate helps care managers use their time more efficiently and helps patients avoid the use of high cost resources such as visits to the emergency room.
Croegaert added that the tool would be particularly useful for the annual wellness visit and five-year care plan that Medicare now requires.
Navigate can also incorporate non health care-related data about patients, such as whether they have Internet access or their own transportation, information that enhances health care providers’ interactions with patients to create better outcomes.
