Netsmart’s care coordination solutions are a key facilitator for quality care in a variety of settings in multiple states, including powering Health Homes and other care coordination models for people with chronic conditions, seamlessly facilitating referrals between providers, and facilitating queries between providers and a statewide Health Information Exchange (HIE).

CareManager™: Game Changing Care Coordination and Data Sharing

The Netsmart CareManager solution enables Health Homes, ACOs, Patient Centered Medical Homes and other care coordination organizations to facilitate coordinated care. Participating providers can follow a shared care plan, exchange consumer data, track clinical quality measures, and manage authorizations and claims across the network. Core to the system is contextual awareness identifying which patients may be at increased risk, ensuring optimal outcomes while improving efficiencies. This comprehensive view into a patient’s health record provides broader insight and visibility into the activity taking place across the care continuum. This allows providers to ensure patients receive the appropriate care at the appropriate location at the appropriate time, while improving quality of care and outcomes and reducing costs.

CareConnect™: Game Changing Connectivity and Integration of Healthcare

The Netsmart CareConnect solution expedites integrated care by connecting providers with patient information faster and more efficiently.

The CareConnect solution allows providers to register patients with a network, create referrals for patients that need to move between care providers, request information from other providers, and receive that information quickly in a standard Continuity of Care Document (CCD). Providers only need to maintain a single connection to the service, and the information they receive is always delivered in the method they prefer.

To date, the CareConnect Network has transmitted millions of records between thousands of organizations. The network facilitates referrals and a multitude of care transactions, furnishing providers with value-based connectivity to relevant clinical data across the care process.

Red Rock Behavioral Health Services, one of Oklahoma’s largest community health providers, received a 68 percent return rate on queries to the state HIE through the CareConnect solution, and provided quality care for clients who were previously out of the boundaries of mental health providers.

Two Tampa, Fla.-based behavioral health providers and 2-1-1 Tampa Bay Cares, Inc., the local health and human services assistance hotline, are now referring local residents needing behavioral health services more quickly and efficiently. 2-1-1 Tampa Bay Cares, Inc. can electronically send patient referrals to both behavioral health provider organizations. The two providers, Suncoast Center, Inc. and Personal Enrichment through Mental Health Services (PEMHS), can coordinate referrals between each other.

Game Changing Data Leveraging

The importance of benchmarking is growing rapidly as providers adjust to a shift from fee-for-service to value-based payment models. Netsmart is helping behavioral health organizations turn information into action through the industry’s largest benchmarking initiative. By giving Netsmart clients access to more than 400 clinical, operational, and financial benchmarks and applying powerful clinical, financial and operational analytic tools to aggregated data sets, Netsmart helps organizations optimize outcomes and drive operational efficiencies.

In addition, hundreds of organizations across the country are contributing de-identified data towards benchmarking and analytics services that aggregate data to provide best practices for the health and human services community. This is an agnostic service supporting organizations regardless of their EMR/EHR solution.

Game Changing Coordinated Care Leadership

To address the alarming statistic that people with serious mental illness die an average of 25 years earlier than the general population due to untreated co-occurring physical health issues, Netsmart is engaged in initiatives that link behavioral health and acute care..

Netsmart is collaborating with Epic to accelerate clinical information interoperability between physical and behavioral health providers and improve coordination across all modalities of care for people with mental illness. Initial participants in the pilot program include Memorial Healthcare System, the second largest public healthcare system in the nation, and Henderson Behavioral Health, south Florida’s oldest and largest behavioral health system.

Netsmart recently participated in a HIMSS14 Interoperability Showcase demonstration with nine other major healthcare technology providers, including Epic, Mayo Clinic, GE Healthcare, Optum and Surescripts. The participating organizations demonstrated a working model of query-based access to a patient’s current medical information at all points in the care.

Netsmart also is a founding member of Carequality, a new industry-driven initiative designed to accelerate progress in health data exchange among multi-platform networks, healthcare providers, and EHR and HIE vendors. Netsmart is the only founding member focused on providing post-acute health information technology to behavioral health providers.
