Loopback Analytics to Provide Technology Platform for Florida Atlantic University’s INTERACT™ Program

Solution manages patient populations and care transitions in real-time

DALLAS--()--Loopback Analytics, a care transitions management platform provider, has established a license to integrate INTERACT™, a quality improvement program developed by the nationally renowned experts at Florida Atlantic University. INTERACT™ (Interventions to Reduce Acute Care Transfers) focuses on the management of acute change in patient condition at skilled nursing facilities across the nation.

The Loopback Analytics cloud-based platform helps skilled nursing facilities using the INTERACT™, program manage patient populations and care transitions by improving tracking and root cause analyses for hospitalization rates, as well as improve communication with local hospital providers. In addition, new tools have been included to enable proper medication reconciliation for medication orders when patients transfer from a hospital to a facility.
