BURLINGTON, Mass.--(BUSINESS WIRE)--Arcadia Healthcare Solutions, a leading provider of data-driven quality improvement solutions for health systems and health plans, today announces that four of Ellis Medicine’s primary care locations have received Level 3 Patient-Centered Medical Home (PCMH) recognition from the National Committee for Quality Assurance (NCQA). Level 3 recognition is the highest achievable recognition level for a medical group. NCQA’s Patient-Centered Medical Home program recognizes physician practices that prioritize the strengthening of the physician‐patient relationship, coordinate care for patients across multiple settings, and engage in a team approach to improve patient care.
“A ‘Medical Home’ is a team approach to providing personalized, comprehensive healthcare for each patient,” said Dean Limeri, MD, FACP, FAAP, medical director of Primary Care at Ellis Medicine. “Putting patients at the center of their care and encouraging them to play an active role on their care team is critical to achieving better health for patients and our healthcare system.”
Applying for recognition is a complex and time-intensive process, often taking practices with limited staff time and resources more than a year to complete. Arcadia supported this effort by providing on-the-ground coaching and technology tools needed to deliver effective, efficient team-based care. Guided by Arcadia’s PCMH transformation management tool, the PCMH Accelerator, and the Arcadia Analytics platform, the Ellis practices received in-practice coaching to define and implement policies and procedures outlined by the guidelines as set by NCQA.
"We are thrilled to have the opportunity to support a forward-thinking organization like Ellis through the PCMH transformation process," said Michael Meucci, director of transformation and improvement at Arcadia Solutions. "We have been working with organizations across the country, making recognition more manageable, and supporting data-driven transformation in the practice. Arcadia not only guides providers through the process of transforming into a PCMH, but also minimizes the overhead associated with transformation, allowing care teams to focus on what they do best: deliver improved care quality and outcomes to their patients."
