WALTHAM, Mass. & LONG ISLAND, N.Y.--(BUSINESS WIRE)--East End Health Alliance and its member Hospitals, Eastern Long Island Hospital, Peconic Bay Medical Center, and Southampton Hospital, have entered into an agreement with MedVentive to provide East End Physician Hospital Organization (PHO) on Long Island, New York with MedVentive Population Manager™. Formed by East End Health Alliance, East End PHO is intended to create a clinically integrated system of healthcare delivery for residents of Eastern Long Island. The group, whose startup costs are funded by a federal grant, seeks to improve healthcare outcomes while reducing costs.
East End PHO has chosen MedVentive Population Manager™ to allow its participating providers, once the system is satisfactorily implemented, to share information at the point of care, monitor performance against evidence-based medicine and best practices, and provide real-time feedback to physicians. The group will initially collaborate on the care of diabetic patients and later expand to the treatment of other conditions.
“With MedVentive Population Manager, our physicians and Alliance hospitals will be able to share clinical information across the entire spectrum of healthcare for the first time,” said Scott Sheren, M.D., managing partner for East End Eye Associates and co-chair for the East End PHO.
MedVentive Population Manager supports clinical integration and population management. With its point-of-care registry, MedVentive Population Manager offers robust data acquisition and aggregation, provider and patient attribution, point of care decision support, and quality reporting (including PQRI). Care management workflows and patient outreach tools streamline provider tasks, distribute work among the care team, and facilitate patient follow-up communications.
“There’s no one better than physicians, who are in front of patients at the point of care, to implement higher quality and more cost-effective healthcare,” said Dr. Jonathan Niloff, MedVentive founder and CMO. “MedVentive is pleased to be working with a forward-thinking organization such as East End PHO. They understand that actionable information, communication tools, and performance feedback will guide their providers toward better patient outcomes at lower costs.”

About MedVentive
While healthcare was evolving the concept of an accountable care organization, MedVentive was busy operating as one. MedVentive was founded as the Provider Service Network (PSN) in 1997 by the CareGroup Healthcare System, an integrated, at-risk network of 4,500 physicians and eight hospitals that cared for more than 450,000 at-risk patients. The network included the Beth Israel Deaconess Medical Center, a teaching facility of Harvard Medical School.
Our origins as an ACO give MedVentive a deep understanding of provider organizations and the healthcare environment. Having been an at-risk provider network, MedVentive is uniquely qualified to support an organization’s transition from a fee-for-service payment environment to risk-based contracting, and provide the clinical integration/population management infrastructure necessary to be successful under this new model.
