Humana and Integrated Health Network Announce Accountable Care Agreement to Improve Seniors’ Health and Wellness

A leading Medicare Advantage provider and the state’s first multi-system accountable care network collaborate to bring more efficient and effective health care to patients

MILWAUKEE, Wis.--()--Improving wellness and driving more coordinated care for Medicare Advantage members in Wisconsin are the primary objectives behind the recent three-year Accountable Care agreement between Humana Inc. (NYSE: HUM) and Integrated Health Network of Wisconsin (IHN).

The agreement, effective as of January 1, 2015, will restructure health care delivery to better control costs and increase standards of care for seniors in Wisconsin.

Humana’s Medicare Advantage members will have access to IHN’s broad range of facilities and physicians in Wisconsin. IHN’s growing clinically integrated network currently includes 42 hospitals, 500 clinics and more than 5,000 physicians spanning northern, central, western and eastern Wisconsin.

The foundation of the new agreement is based upon Humana’s Accountable Care Continuum. The continuum is a stepped approach to pay-for-value reimbursement that allows the company to implement different value-based partnerships with hospitals and doctors based on their infrastructure and value-based reimbursement capabilities.

The partnership will also offer more personalized care and a broad range of population management tools like predictive analytics, chronic disease management and wellness programs. The focus will be on care coordination and leveraging measures as defined by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS) like diabetes care and treatment, breast cancer screenings, colorectal cancer screenings and high-risk medication.

Humana has a 27-year accountable care relationship history with more than 1.3 million MA members that are cared for by 42,300 primary care physicians, in more than 900 accountable care relationships across 43 states and Puerto Rico.
